SVSR: Supplement-Drug Interactions, Exercise & Your Pysche, Running vs. O-Lifting vs. Heart Health, N3-to-N6 Ratios, CYP Enzymes, Cannabinoids & Telomeres

The "Seconds" are as the name implies no "leftovers", but actually yet another selection from the selection of god knows how many interesting newsbits I usually pile up for the short 1h show, Carl and I are doing every Thursday. I would therefore encourage everyone to do both, listen to the podcast and read the "Seconds" one day later. After all, the things Carl and I discuss on the air won't reappear here, they are "SHR exclusives", so to say ;-)

Apropos, in yesterday's show, the topics we did cover were
  • premature ejaculation, and how only two hormones seem to make a difference,  
  • peptides as prostate cancer vaccine, and how Harvard scientists build them from scratch,
  • supps vs. medications, and how fatal commonly overlooked interactions can be, and
  • copper, and why it may well matter than raw milk has 2-3x more than pasteurized milk
and before we go on with the actual "seconds", I must acquit myself of a promise -- the promise to provide you with more information on #3 on the above list.

Supplemental data: Supplement vs. drug interactions

Those of you who have already listened to the podcast will probably be waiting eagerly for the supplemental material with more information about the potential pitfalls with supplement-drug interactions, Carl and I have been talking about on yesterday's show. With some digging, typing, searching, excerpting and formatting on my part, I have actually come up with a quite comprehensive and for people who are not familiar with all the funky drug names, probably even more understandable version (see figure 1) of the tabular overview H.H. Tsai and colleagues from the China Medical University Hospital and the College of Pharmacy at the University of Illinois at Chicago have included in their latest review of the literature (Tsai. 2012).

What I left out are the two pages (!) part on St. John's wort. With 147 drug (!) interaction ranging from "A" as in "amiodarone" to "W" as in "warfarin" and covering almost every drug type from anti-depressants, protease inhibotors, calcium channel blockers, PDE-5 inhibitors (viagra & co), SERMs, proton pump inhibitors, etc.. In view of the fact that these are only the known interactions, it would be easier to list those drugs with which St John's does not conflict, anyway. So, unless you have a study at hand which conclusively shows that St. John's is no problem, I would rather err on the side of caution, than end up in the ER.

Top 5 of the most frequent interactions observed with medication that act on (ranked by frequency, figure in brackets indicates percentage of all drugs in the study; based on Lin. 2012):
  1. nervous system (19.6%)
  2. cardiovascular system (17.7%)
  3. antiinfectives for systemic use (14.7%)
  4. alimentary tract and and immunomodulating agents (12.2%)
  5. musko-skeletal system (6.4%)
As far as the supplement list in figure 1 goes, the most frequent potential side effect due to supplement-drug interactions affected drugs / supplements that play a role in blood coagulation. Danshen, evening primrose, gingko, glucosamine, white willow bark, garlic, vitamin E, fish oil to name only the most common ones, they all can increase the risk of bleeding not only, but specifically in patients who are taking warfarin (aspirin, ibuprofen, heparin and others were on the list, as well).

"What’s wrong with telling a patient, 'If you don’t hear from us with your lab results a few days, give us a call'? The answer is plenty, if that patient is receiving warfarin therapy. Because warfarin has a narrow therapeutic range and complex pharmacology, insufficient monitoring or errors in dosing can lead to severe and possibly life-threatening bleeding and clotting in patients receiving it." (Bush. 2002)
In view of the "top 5" above, this certainly sounds counter-intuitive, but we are dealing with a practical research bias here. As I mentioned on the air, there is simply an overabundance of research on potential interaction with warfarin, because finding the right dosage and adapting it appropriately is already hard even when there are no confounding variables, so that a sudden supplement-drug interaction and subsequent increase in the risk of bleeding can potentially be fatal (see the quote in the red box to the right)!

Regardless of what medication you may be on, rules that apply for a healthy individual that does not take any medication chronically (not even 'harmless' NSAIDs), don't apply to you! So please for one, follow the recommendation you find on each and every supplement to "talk to your medical practitioner" before you add another 'harmless' supplement on top of the 'harmless' over-the-counter or prescription drugs you are taking.

The Seconds: Interesting news that have been missing from yesterday's show

After this pretty lengthy addendum, let's get to three other items I had actually planned to have on the show, two of them are exercise, while the third one is a health and supplementation... and, when I come to think about it, obviously also diet related news-item:
  • Exercise makes you happy and puts an end to the greed for money! That's not exactly the result of a recently conducted study from the Charité in Berlin (Bothe. 2012), but it is more or less what follows from the differential response Bothe et al. observed in their untrained and highly trained subjects to monetary stimuli after they had completed a standardized running exercise (30 min at 60-70 % VO2max, T) or placebo (P).
    Who would have thought that: Exercise reduces the anticipatory response to monitary incentive delay (MIT) test (Bothe. 2012)
    "Acute exercise was found to influence gain anticipation. In the P compared to the T group a more pronounced anticipation-related BOLD response was found in mesolimbic and mesocortical dopamine-innervated regions like the VS, hippocampus (Hipp) and subgenual anterior cingulate cortex (sgACC). [...] Additionally, several brain structures potentially associated with motor preparation (primary and supplementary motor areas) as well as structures belonging to the ventral (lingual gyrus) and dorsal (cuneus, precuneus) visual pathway showed stronger BOLD responses to gain anticipation in the P group compared with the T group." (Bothe. 2012)
    Moreover, according to the paper which is going to be published in one of the upcoming issues of Medicine and Science in Sports and Exercise, all 43 healthy men between the age of  20 - 32 years who participated in the study showed similar increases in mood (effect size F=11.70).

    With both, the beneficial outcome of the positive and negative affect schedule and the decrease in anticipatory signalling (= the greedy "I am about to win!") in the psychological testing session (the so-called monetary incentive delay) in an fMRI brain scanner, being identical it becomes evident that you don't have to be an athlete to monetize (all puns intended ;-) on the beneficial psychological effects of exercise.

    Changes to the heart (right ventricular) due to exercise are mild, and if anything more pronounced in response to endurance than resistance training! " Left ventricular (LV) adaptation to exercise training has been the focus of 'athlete's heart' research to-date, information regarding right ventricular (RV) adaptation is sparse, due to its complex structure and imaging technique limitations." (Spence. 2012) So scarce, in fact, that this recent study that has been conducted by researchers form the The University of Western Australia, the University of Leeds, a and the Liverpool John Moore's University is the first to take a closer look at the impact endurance or resistance training have on the morphology of the RV.

    For their randomized trial, the researchers recruited twenty-three young untrained men.. The men were assigned to either
    • endurance training (E; n = 10)  - consisting of a progressively overloaded program of walking/jogging/running, divided into three training phases over the 24-week period, or
    • resistance training (R; n = 13) - with a focus on periodised R program was Olympic weightlifting with incorporated assistance exercises (e.g. deadlift, squat, bench press, overhead press) to develop overall strength and technique
    for a total timespan of 6 months, in the course of which body composition, aerobic fitness, muscular strength, RV morphology (MRI) and function (speckle tracking echocardiography) were continously monitored.

    The results Spence et al. are going to publish in one of the future issues of Medicine and Science in Sports and Exercise refute even two pieces of common "knowledge". Firstly, a still totally benign, right ventricular hypertrophy was exclusively observed in the endurance training group, yet not in the heavy lifters who were doing their squats, deadlifts and military presses (by 2.7g following E and by 1.4 g  following R training). Secondly, both strength and size gains were no prerogative of the lifting weight group. Contrary to the increase in total lean mass (+1.3 kg vs. +2.1 kg), the strength increase of +53.8 kg vs. +35.3 kg was yet much significantly more pronounced in the weight lifters.

    On the other hand only the endurance training group saw significant statistically improvements in their aerobic fitness level. This correspondence of endurance exercise, mild ventricular hypertrophy and increased fitness levels is unquestionably telling in terms of "how bad" a physiologically enlarged heart where the ratio of left-to-right ventricular  mass remains intact (which was the case in the study at hand), don't you think so?
  • Omega-3s, omega-6s, telomere length, CYP enzymes, endogenous cannabinoid and the liver you need all of them to see the complete picture While the epidemiologists are still debating who will and who won't benefit from omega-3 supplementation, those who still care about how our bodies works and why their colleagues over at the epidemiology department are still debating, have made quite some progress as far as the underlying health benefits of rectifying the omega-3 to omega-6 balance are concerned.

    Why are endocannabinoids problematic? One of the answers is: "They will make you fat!" Basically we have known that forever, but a recent study which tracked the conversion of dietary linolic acid (n-6) to it's endocannabinoid metabolits, 2-AG and anandamide has recently confirmed not just that, but also that the provision of no more than 1% of the total energy of the diet in form of eicosapentaenoic acid (EPA) + docosahexaenoic acid (DHA) can already make a huge difference (Alvhem. 2012). With the addition of the long-chain omega-3s, the rodents in the study had a 8:1 ratio of linolic acid (LA) to long-chain omega-3 fatty acids in their diets. Still much higher than what you will hear is necessary, but sufficient to reverse the overabundance of arachidonic acid, in the phospholipids of liver and erythroctes, and the +200% increase in endocannabinoid levels that had been brought about, when the researchers had increased the linolic acid content of the diet from 1% to 8% of the total energy intake. In view of the fact that the same goes for the increased food intake, feed efficiency, and adiposity the mice had developed on the 60% fat (total) diet with a high linolic acid content, this study - despite being done on rodents - clearly shows that it does not necessarily have to be a 1:1 ratio to grasp major health benefits.

    If you get down from  30:1 to 8:1 you've come a tremendous way, already; and guess what: The easiest way to achieve that is to just cut out all seed and vegetable oils as well as processed foods that contain them.
    In a recent review on the differential effects of fatty acids on human metabolism in the Italian journal Medical and surgical pediatrics G. Caramia emphasizes the role of omega-6 derived endocannabinoids:
    "[E]ndocannabinoids like anandamide (N-arachidonoylethanolamine) and 2-arachidonoylglycerol [that arise from the enzymatic conversion of linolic acid by enzymes from the cytochrome P450 family at the liver are] capable of mimicking the pharmacological actions of the active principle of Cannabis sativa preparations such as hashish and marijuana (-)-Delta9-tetrahydrocannabinol. They act as true 'endogenous cannabinoids' by binding and functionally activating one or both [of the] cannabinoid receptor present on nervous and peripheral cell membranes." (Caramia. 2012; my emphases)
    Unfortunately, the same enzymes which are responsible for the generation of those endocannabinoids, are also responsible for the conversion of n-3 PUFAs into more potent metabolites of EPA and DHA, which will actually do most of the the vascular- and cardioprotective magic that is commonly ascribed to "fish oil".

    And how does all that relate to telomeres?

    These competitive effects in turn segue directly into the observations of a double-blind 4-month trial that involved 106 healthy sedentary overweight middle-aged and older adults. The participants supplemented their diets with capsules containing either (1) 2.5 g/day n-3 PUFAs, (2) 1.25 g/day n-3 PUFAs, or (3) a placebo that mirrored the proportions of fatty acids in the typical American diet.

    Now, it's not news that this led to decreases in inflammatory markers. I am not going to bore you with those, don't worry!

    What is news, and in my eyes very important, is that neither the provision nor the dosage of additional long-chain omega-3s had an effect on telomere length, the only variable that mattered was were the changes in the n-6:n-3 PUFA plasma ratios, which "helped clarify the intervention’s impact: telomere length increased with decreasing n-6:n-3 ratios (p= 0.02)" (Kiecolt-Glaser. 2012).
That's it as far as today's seconds go... you want more? Man, I could certainly give you more, but you know that gluttony was once considered a sin, right? Conventional vs. Organic: It's Not About Getting More, But Getting Less For Your Money. Less Pesticides, Dioxins & Co

Image 1: The Ökomonitoring program took a very different approach to the question whether or not organic products are worth the extra cost, they did not look for the additional edge, but rather the "subtractive edge", if you will, and tried to answer the question: "Can we even produce 'organically' in an increasingly polluted environment"; now ten years after the program started the answer appears to be "Yes!" (MLR. 2012a)
"Organic produce is no better than conventional! You are being ripped off!" Statements like this have made the headlines in the past couple of days and many people (obviously not you, though!) are scandalized or shaking their head haughtily uttering things "I knew it, and that's why I always buy the cheapest..." What both groups are missing, though is the simple fact that the question the scientists posed, or rather the question the journalists put to the forefront, i.e. "Are organically produced fruits and vegetables more nutritious?", is nonsensical, because it would imply that the absence of certain chemical agents and production methods would increase the amount of nutrients a plant puts into its fruit, leaves, roots or whatever. Since I have discussed this with my friend Carl Lanore on yesterday's joint SHR + SuppVersity News Round Up pretty extensively (in fact so extensive that we could not cover the other topics I had lined up - sorry for that, folks!), I won't go into more detail on that here, but simply repeat that the fallacy of this approach was part of my reasoning not to bore you with write-up / comment #5325 published that's been published on the bazillion of blogs within the past couple of days, but rather give you the long and short of a governmentally subsidized program that's been running for 10 years, now, here in Germany: the Ökomonitorin program of in Baden-Württemberg (this is one of the individual states here in Germany).

It's not about what's in there, but about what is not!

The researchers who are mainly working at the University of Stuttgart set out with a whole different research question than most of their colleagues. Rather than trying to answer loosely defined questions such as "What's better: Conventional or organic", they wanted to know whether or not it would even be possible to "produce organic" in an environment that is already profoundly polluted; and though I don't want to give away too much in advance, after 10 years and ten-thousands of samples of organic and conventional fruits, vegetables and animal products being analyzed the answer is "Yes it is!"
"Organic fruits and vegetables had on average 180x lower pesticide content than conventional products; and only 5% of the samples from organic produce were objectionable."
That's the conclusion the researchers in the 10-years special report that has been published in July 2012 (MLR. 2012b). Since I know that you don't content yourself with universalities like this (otherwise you would hardly be here ;-), I would like to invite you to let me walk you through some of the detailed results of the latest data from 2011, although this may not be the exact same goods you have (or haven't) been buying in the course of the last year (I just checked: Only ~6% of you are actually from Germany)

The good the bad and the equally ugly

While the general trend that emerges does in fact speak in favor of the organic products, a brief look at the data in figure 1 should suffice to see that despite the fact that the number of offenders, i.e. products with pesticide residues above the maximally allowed threshold is close to zero, even organic produce is far from being pesticide free.

Of all the categories I picked when I compiled figure 1 there are however four product categories that caught my eye immediately, because the "organic advantage" as you may call it, is particularly small, here: Wine, plant oils and processed fruit and vegetables (the latter category included frozen, usually relatively pesticide free, but also canned food and dried foods, as well as fruit jars etc.). Intriguingly, in all of those four the pesticides could (and in the case of the Wine, the researchers even proved that) have gotten into the food-chain during the processing / manufacturing process, as well - in other words processed and organic tend to be contrastive pairs.

When it comes to the actual "offenders", i.e. those products with levels above what is officially allowed, here in Europe, there are actually seven of which I feel that they really stand out (one for each day of the week, isn't that great?):
Mothercorn, the "abortion fungus" of the middle ages, the first source of LSD and the result of inappropriately stored and subsequently moldy grains / flour being used in the production of breads, is a potential cause of auto-immune diseases and lethal toxicities.
The mothercorn (Claviceps purpurea) content [µg/kg] of multi-grain breads with rye and wholemeal rye bread (please mind the leap on the primary axis; data based on MLE. 2012a)
The ingestion of 2 mg of mothercorn does already lead to adverse reactions which range from nausea over headaches, and cramping to a loss of sensation in the extremities and vascular occlusions (the latter will also arise from chronic low-grade intoxications)- in pregnant women it will induce contractions of the uterus and spontaneous abortion. The ingestion of 5-10g of the ergot-alkaloids from the fungus kill an adult man... not exactly what you want to have in your bread, right? Not even in "harmless quantities" - which is what even the worst offenders would have to be considered, still.
  • the processed foods - irrespective of whether you buy veggies or fruit, you can be sure that the contamination raises with every processing step; be smart not lazy and buy whole foods, learn how to prepare them and cook for yourself
  • cereals - 3.4% of the "oh so healthy superfoods", of which at total 72% of the conventional and 36% of the organic ones contain pesticides, are also on the list of items even the European officials suggest you should better not eat, if you don't want to become sick
  • legumes - another favorite of mainstream dietitians are not just among the worst offenders (24% of the conventionally produced legumes contain pesticides above the tolerable limit), they are also the only product where even the organic variety is not suitable for human consumption in 11% of the cases and as if that was not enough, the number of substances above that threshold amounts to 17 compounds in the conventional and 3 substances in the organically produced legumes
  • leafy greens - the health food per se may not be among the worst offenders but in those 6.7% of the samples that had pesticides above the the threshold level, the scientist were able to identify 27(!) different pesticides (note: not all pesticides were found in all samples and the worst offenders were not broccoli & co, but rather salads)
  • ginger (=ingwer)- the conventional variety something many people use to improve their health had higher than tolerable levels in 12.5% of the cases; a health food "gone unhealthy", if you will
  • berries - they may not be among the worst offenders, but they are "versatile": With 5 different pesticides in the samples the scientists analyzed they can't compare with the processed veggies with 45 different pesticides, but are doing quite "well"
  • exotic fruit and citrus fruit - while with them the conventional farmers obviously assume that the consumer would throw away the peel anyway; or, and this segues right into our next topic, there are no governmental controls and regulations in place; after all, you should be aware that Germany is not exactly the country to grow "exotic fruits" ;-)
If we seize on the notion of "it's not us, it's only the others that contaminate their produce, ship it to Europe (or the US) and poison us", we must however realize that things aren't so easy as they seem: While the German organic produce may in fact be the less polluted one, neither the Israelis nor the South Africans or Moroccans, but rather our European friends, the Italians are trying to poison us ;-)

Now, all jokes aside, it is simply not possible and probably also highly un-rewarding to replace one prejudice, i.e. all organic produce is good with another more sophisticated one such as all organic produce from Germany is good and everything from Italy, Egypt and Greece is poison. Therefore, it is probably also not so important that the intricacies of these statistics are meaningless for the majority of you who are (I just checked) US residents and not among the 6% "resident" (=German) SuppVersity students ;-)

Organic = Say no to GMO!?

With the different legislative situation here in Germany, the quantitative data on GMO "contamination" in figure 3 is probably likewise irrelevant for many of you, but with the production of GMO corn being banned, here in Germany, it does give you a sneak peak on what can happen if the government is willing to act on behalf of its people (the majority of Germans is against GMO) instead of the industry (in all fairness it must be said that it is the US and not the European, let alone the German agrar-industry that's constantly trying to push GMO corn and other produce into the German market).

Even among the conventional produce, the number of outliers was small - roughly 5% of conventional products contained <0.1% corn from genetically modified plants - a result which is probably also to be accounted for by the awareness of the industry that they will have to throw all their produce away once the GMO-skeptic consumers realize that product is contains significant amounts of genetically modified plants.

The data in figure 4 does yet also show that things look different for soy products, most of which are imported from countries where the production of GMO foods is actually allowed (like the US ;-) and even non-GMO crop can be exposed to cross-contamination. If we take a closer look and discard the annual variations, it does yet become obvious that the GMO content of organic products is just as their pesticide content still much lower than it is in conventional ones. What is pretty troublesome, though, is that in 2011 25% of the products that are advertised as "GMO free" contained small amounts (<0.1%) of genetically modified soy - for you, my American friends this means: Having a label alone does not protect you from "GMO fraud" ;-)

Eggs, salmon, animal productts and other notable observations and changes from 2002-2012

Before I'll conclude this post with a couple of notes on trends and general observations the scientists who have been working on the Ökomonitoring program over the past 10 years made and let you make up your own mind of "whether or not you feel that getting less is worth paying more", let me briefly address the issue of dioxin-laden organic eggs Carl and I have been talking about, yesterday, already:

Plus: Domestic vs. Foreign - What's Healthier?". After all, your own organic tomatoes and other produce can be effected by the environment in which you grow them, as well.
  • Not just organically produced eggs, but also organically produced salmon have on average higher levels of DDT, PCB and total dioxins (TEQ) and HCB, DDT2, PCB 153, Dieldrin, Endosulfan, Chlordan, Toxaphen, Tribromanisol, PBDE, respectively (note: wild salmon is almost free of any of the latter!)
  • Those contaminations are not brought about by what the farmers do, but rather what they didn't do, i.e. make sure that their free ranging hens are not running around on toxic soils and their fish are not swimming in a toxic sea - and this is where the initial research question of the Ökomonitoring program, "Can we even produce organically?", must be answered with a somewhat shabby: "It depends" and the one parameter it depends on is the environment, to which the animals (and to a much lesser degree, the crop) are exposed
Now, the implications of this are not that all organic animal products are worse than their conventional counterparts (in fact the number of products without any residues is larger for the organic products, unfortunately those products which are contaminated contain much higher levels!), but rather that you have to be choosier and try not to rely on the label "organic", if you know your local farmer and trust him, you may be better off buying his non-organic eggs that those from an organic farm on contaminated land...

If you type in "Bronx New York", for example, you will learn that the major risk factor from air pollution is formaldehyde, that there are two final NPL sites and so on and so forth (thanks to Carl Lanore and Alisa Profumo for pointing this out on today's Casual Friday).
But enough of that let's wrap it up for today, with a couple of more or less unrelated, but interesting facts, the first of which is so important that I decided to repeat it, although I mentioned it at the beginning of the article, already:
What is Furan? Furan is a heterocyclic compound that has originally been used in the chemical manufacturing industry. It does however also occur during the combustion of coal and is a component of tobacco smoke, as well as in a number of heat processed food items such as canned and jarred foods and coffee (FDA. 2004). Furan is carcinogenic to rats and mice and was classified as ‘possibly carcinogenic to humans’ (International Agency for Research on Cancer, 1995; see as well Cordelli. 2010; Chen. 2012)
  • on average 180x lower pesticide residues in organic fruits and veggies - that's the astonishing result after 10 years of Ökomonitoring
  • no = ZERO pharamcologically active substances in any of the testes organic produces
  • antibiotics residues in organic honey were reduced from 23% to 0% from 2003 to 2005; no complaints thereafter
  • the mycotoxin load is about equally high in conventional and organic produce
  • No significant differences in terms of organic contamination and pesticide residues exist between animal products; the dioxin load in eggs free ranging hens and organic salmon are exceptions to this rule (while levels spiked in 2009, when "buying organic" really took off over here and more and more farmers started producing "organic eggs", the levels have been slightly declining over the past years)
  • the way organic coffee is roasted leads to increased furan levels in organically produced roast coffee
  • the amount of acrylamide in cookies is equally high in organic vs. conventional products, the in organic chips is higher and the number of samples with levels above the 1,000µg/kg threshold is higher as well
  • no differences were found in terms of trans fatty acids in ready-made meals
  • organic softdrinks are free of preservatives
  • no artificial colorings in organic sweets
Now it's up to you - organic, yes or no? And remember: We cannot always make everything 100% right, but that does not mean that you should give up trying.

Follow Up on Set-Point Theories: Could Royal Jelly Help With Obesity / Diet Induced Structural Brain Changes?

The colleagues over at ergo-log.com dug up a 2009 study (Narita. 2009) on the endocrine effects of royal jelly supplementation on rats. You may now wonder "Why is he suddenly beginning to writing off information about old studies on other sites?" Well, my interest in this study relates to my recent appearance on Carl Lenore's Super Human Radio show (I know, I promised a write-up /just have to find the time ;-) and diverges from the original interpretation the anonymous author over at ergo-log provided.

As you may have heard on the podcast, the most significant finding of the Ravussin study (Ravussin. 2011) was the down-regulation of pro-opiomelanocortin [POMC] neurons in the hypothalamus, which went hand in hand with weight gain and weight loss and is associated with a reduction of the metabolic rate to levels at which further weight loss or even weight maintenance become a significant challenge. Here, the findings of the aforementioned study on royal jelly, "a honey bee secretion that is used in the nutrition of larvae, as well as adult queens" (Wikipedia), come into play. As can be seen from in figure 1, 7 months supplementation with 5% Royal Jelly produced profound effects on the expression of the rats' pituitary hormone genes, one of which (you will have guessed that) is POMC.

Now, one could speculate that despite the fact that a ~10% increase in POMC expression could not compensate for the obesity induced loss of 50% of excitatory POMC neurons observed by Ravussin et al., the combined effect of increased POMC expression and a rise in thyroid stimulating hormone TSH could eventually turn out to be beneficial for someone, who has maneuvered him or herself in a metabolic state, where effective weight loss, i.e. the loss of body fat and conservation of muscle mass, is nigh on impossible.
Yet, before you break into the next beeyard, you should consider that...
  1. the amount of royal jelly the rats consumed in the study, i.e. 5% of their daily food intake is almost impossible to achieve without plundering beeyards all over the country (for the same reason it is no practical alternative to medications for "rejuvinating the pituitary" which the author of the ergo-log post seems to imply in the headline of his post)
  2. an upregulation in gene expression alone does not guarantee an increase in metabolic rate (the increase in the thyroid (pre-)hormone T4, for example, was non-significant, despite a significant increase in TSH related gene expression)
Basically, this means that you should get accustomed to the idea that the damage years of overeating and/or crash- & yoyo-dieting has inflicted to your body are unlikely to be repaired within weeks or even months by just popping pills or eating some kind of superfood, no matter how "royal" it may be. In the end, it will come down to making a decision to change your life (nutrition and exercise) and to stick to this decision, no matter how hard it may seem.

"The Good, The Bad, and the Ugly"

If you’ve ever seen the movie, that’s pretty much how I you could sum up the events in Vegas. As wild, and fun as it was, it also got pretty ugly towards the end. Someone broke into our hotel room early Sunday morning while everyone was still out and ... no, not training, but partying ;-) And when we finally came back to the hotel at around 7 am, we were so intoxicated that we didn't even realize that we got robbed. Only after we all got some shut eye, and woke up a couple hours later as we started packing our stuff to check out, one of my boys realized that someone must have had opened his bag and after had taken his iPod.

It took us only a few seconds to realize that all our bags had been searched through, and it struck me like a 200lbs dumbbell, when I went I looked towards the table, where my MacBook had been sitting and saw ... nothing. The damn thing was Gone! And with it 4 years of my life's work. From school stuff, research papers, personal projects, client portfolios, pictures, etc. that and much more, it was all on the hard-disk of that damn machine. What really saddened me most, though, was that my whole documentation. All the photos, nutrition and training plans, I have been documenting since late summer for my 2012 contest prep, all that was gone. I had kept an in-depth journal documenting my 2012 contest prep. It had everything from progress pictures, stats, calorie intakes, macros… Everything Gone!

I could care less about my laptop, materialistic things can be replaced, but there was a lot "in" that laptop that had a lot of sentimental value to me… well, I guess this is a case of "another one of life’s lessons learned". And if there is anything that I have learned since my spinal cord injury, it is that you can't cry too long over spilled milk. You got to stop thinking about problems and begin to work on solutions. In my case this does unfortunately mean that I have to start all over again, while trying to recover as much information as I can from the last backup I have (about 50% of the data), Facebook, personal and professional email transactions and obviously my blog, here at the SuppVersity. If I am lucky I will thusly be able to recover 75% of what I have lost... and most importantly: I able to find was the "before pictures" I took, when I first started my prep...

Vodka, Tequilla and only 8 weeks to go!

A pros pos prep! Just 8 more weeks to go to the big day. My big day at the Wheelchair Nationals in Palm Beach, Florida, and I know what you are thinking now: "8 weeks out and this idiot heads to Vegas and gets so hammered that he doesn't even realize he has been robbed? What kind of bodybuilder is that?" I guess the best answer I can give you would be: "One who is passionate about training, but does not live to train, but trains to live!" And, at least judged by my latest pictures, the weekend with Vodka and Tequila Sunrise instead of protein shakes does not seem to have hampered my progress at all - wouldn't you agree (and remember contrary to political elections, bodybuilding contests are not won by the guy with wit the nicest smile ;-).?

As for how my nutritional regimen went…well on Thursday night, I broke my fast with a couple shots of vanilla vodka, cocktails, a slice of pizza and a set of sexy painted toes attached to a pretty young thing for dessert ;-) [Toes are calorie and carb free with no nutritional value by the way… lol], and I think I had a burger and fries at a local burger joint at the hotel, where we were staying at for Friday and Saturday night. Overall, everything went just as planned... pretty chaotic! But I stuck to my fasting and feeding protocol and made good use of the flexibility it allows for by making a habit of braking my fast with a couple of shots of vodka, instead of food.

Getting back from Vegas and back on track

The Monday after I decided to go on a strict veggies and protein only diet for the next couple of days. I pounded a lot of water, to rid whatever excess water I may have retained during the weekend (although I honestly did not see any) and then reintroduced my post workout carbs on Wednesday night after the workout. It didn’t really hit me how close my show is and that 8 weeks is not that long, so I started playing with my caloric intake and macros again while trying to update everything on my new laptop. When I was reasoning about the macronutrient ratios, I realized how much of a difference the reintroduction of carbohydrates had made in terms of the way I looked and the intensity with which I trained.

When you are getting into the single-digit body-fat realm, small dietary (or supplement) changes often make a big difference.

For me the ~100g of carbs make me feel much fuller and tighter. The increase in glycogen content goes hand in hand with an increase in vascularity and my muscles look overall much grainer. At the same time, I know I have to keep gradually dropping my calories to lean out, if I want to be beyond shredded by show time. I will therefore start to cycle my calories between 1500 kcal to 1200 kcal - with the higher, 1500 kcal intake on workout days, and the lower 1200 kcal on non-training days.

The 10g of carbs are trace carbs from my whey protein; I usually wouldn’t count it but when it gets down to the final weeks, it gives me the peace of mind that is necessary to keep cortisol at bay ;-) All 65g of my PWO carbs will come from organic russet white potatoes (about 55g), and the rest of the 10g I’ll use on light fruits like berries, papayas or pineapples (about ½ a cup to add to my cucumber salad that I make… I still don’t count veggies by the way).

65g of starch carbs is all it takes to let make muscle pop

"Only 65g of starchy carbs?" Is probably something the carboholics out there are just asking themselves!? For me it is plenty to get me full and satisfied. The cucumbers and zucchinis will then do their share to fill up my tummy, while the carbs from the potatoes find their way right into the bellies of my muscle, so that I can even feel the muscle contractions pop, when I flex! An unexpected yet very appreciated side effect, by the way, is that along with increased vascularity, even the excess skin that I can pull in my lower abs magically disappears when the muscle beneath it gets its share of the glucose that is floating through my veins.

The zigzagging, as far as calories goes (cf. workout and non-training days in tables 1-2) is actually an old trick of Vince Gironda. He called it the Hawaiian Diet. The Hawaiian diet consists of red meats, chicken or fish, with either pineapples or papayas. The macronutrient foundation of every meal is protein + fat meal - the exact food combination vary, though. In that Vince deliberately selected pineapple and papaya for their high enzyme content, of which he believed that it would help with fat loss.

The reason this protocol appealed to me is that both the macronutrient composition (mainly fat + protein), as well as the foods you are supposed to eat, are pretty much identical to the interpretation of intermittent fasting I have come up with in the course of the past weeks and months. And although I am repeating myself here, all geeky science, AMPK and mTOR stuff aside (I'll leave that to Dr. Andro ;-), it's major advantage over your usual contest-prep diet is its ingenious simplicity and convenience... 50g of meat, 20g of fresh pineapples, and 15g of raw coconut oil - how much easier could dieting for fat loss possibly get? Having the total caloric intake and the macronutrient ratios in check, I am pretty confident that the caloric zig-zagging and the weekly re-feeds on Sunday will compensate for the overall borderline low calorie intake and prevent my metabolism from slowing down over the course of the next week.

When adaptation occurs, stagnation begins

Obviously, my training will have to complement what I am doing diet-wise and thusly I have revisited the results from the 5x5 routine I have been following as of late and decided that it may be about time to switch to a higher volume / rep range program for some time. Aside from the higher rep ranges I have been using on a few auxiliary exercises, I stuck to the 5-6 rep range for the whole contest prep. And although it has worked fine for now, I start to notice that both the escalating density training (EDT, cf. "Things Begin to Escalate"), as well as the 5x5 training don't feel as intense or tiring as they used to. I have gotten stronger, my stamina has increased and my body has adapted... and while adaptation means progress, it will entail stagnation, whenever you fail to provide new stimuli to which your body will have to adapt. For me, an increase in training volume will hopefully do just that. I will yet still have to figure out how I can make sure that the intensity of my training sessions does not suffer from that. So, in essence, I will try to lift as heavy as I can, but with 8-12, instead of 5-6 reps. The exact training schedule is yet still in the works, but I will have it nailed down before next week.

Want to Stay Lean on a High-Fat Diet? Consume Whey Protein Everyday.

Listeners of Carl Lenore's Super Human Radio already know: Dr. Paul Arciero of Skidmore College is going to publish a study the results of which confirm that obese individuals can lose weight and improve markers of metabolic health by just adding a 20g shake of whey protein 3x a day.

While you still have to wait for the detailed results of this study to be published, another group of scientists (Shertzer. 2011) derived similar results from a study on mice. Despite being on a high fat diet, mice who received 100mg of whey protein isolate (WPI) per liter of their drinking water (WPI group) ...
had lower rates of body weight gain and percent body fat and greater lean body mass, although energy consumption was unchanged. These results were consistent with WPI mice having higher basal metabolic rates, respiratory quotients, and hepatic mitochondrial respiration. [...] Livers from WPI mice had significantly fewer hepatic lipid droplet numbers and less deposition of nonpolar lipids. Furthermore, WPI improved glucose tolerance and insulin sensitivity.

Adelfo Cerame - Road to Wheelchair Nationals '12: Dismiss Calorie Cutting, Pancakes & English Muffins, Embrace Intermittent Fasting, Fruits and Each Step on the Way!

Sometimes its strange how things get together unplanned. With me being on Carl Lanore's Super Human Radio, today at 1pm EST (click here to listen live!), talking about what happens when science and the real world "collide" and how both scientific paradigms and conventional wisdom are probably the most common causes of questionable conclusions (science) and lack of results (real world) and Adelfo sending me his weekly update on how the "wanna lose fat, got to cut calories"-paradigm is still bothering him...

Five weeks out and putting bees into my own bonnet!

I am counting the days, ... mainly because I don’t know what else to do with myself *lol* - It just feels like I need to do something to manipulate my training and my diet even further, but I really can’t do much at this point, but stay the course… I’m trying not to cut any more calories, because I want to avoid either losing muscle or at least running so flat that another "mini-bulk" would be necessary to get stage ready... I must admit, though: It is tough!

I guess it’s just a habit that I have gotten so used to in the past, that the program in my head keeps telling me: "Hey, contest prep. Another week, another cut in calories" It's always been like that in the past. Wanna get leaner? Drop calories! Well, ... at least that's at least what common wisdom tells you and told me. In reality, however, it can - and at some point will - easily backfire. While it is obviously true, that, in order to get lean, and drop body fat, you must manipulate your macros, and gradually reduce calories, but at one point (calorie, or I should say energy-wise) the returns you get (=fat loss) are no longer worth the investments, which is muscle mass, when your cortisol skyrockets and your testosterone and thyroid levels plummet. The further you are pushing from that point, the less fat and the more muscle you lose.

Where this "point of diminishing returns" actually is, differs from person to person and may even change in the course of your lifetime. From what I gather it has a lot to do with what people like to call your "body-fat set point", something I guess, at least some of you may have heard Dr. Andro talk about on Carl Lanore's Super Human Radio back in the day. The basic assumption is that your body has a certain amount of body fat it does (for whatever stupid reason ;-) defend "with its life", or rather your muscle and organ mass. So, if you are hovering towards the "disgustingly ripped zone", like I am, you are already way past the comfort zone, where your body does not really care about the 1-2% more or less of fat on your ribcage.

Don't fix things that aren't broken, just 'cause you like the "tweaking" ...

The fact, that the usual side-effects like constant cravings, lack of energy & libido still have not hit me yet, must - at least in my humble opinion - have something to do with the satiety effect of intermittent fasting. The fasting-feeding-fasting-feeding... I guess Dr. Andro would say "seesaw" *lol* has really allowed me to eat to satiety over the whole 20+ weeks of this otherwise probably unbearably long contest prep.

I mean, there is a reason that the typical bodybuilding prep (at least the hot phase) lasts no more than 2 months. The constant cravings, the insane amounts of cardio the "chicken and rice"-only menus... nothing of that in my diet, which - I know I've said that before, but it is just so f**** amazing - is by far the easiest and most enjoyable prep in my whole bodybuilding career... with a further reduction in calories, however, things could change very easily and thus, I feel that it is better to check my desire to "just try for a few days" (which would be what I would be telling myself, although I knew that it would be a one-way street), if leaving out another 10g of carbs less here and the coconut oil, there...

...STOP! When you are getting to points like that, it is often best to revisit your plan and, more importantly, the reasonings on which it is based and mostly, you will realize that those streaks of "what if's" are largely irrational. For me this means that I will stick (just as planned till ~2 weeks out) to my caloric zigzagging regimen with 1400-1500kcal on training and 1200kcal on rest days and the planned re-feeds on Sunday. After all, I am still making forward progress in terms of the way I look, although I must admit that my objectivity is hugely compromised by the little voice of common wisdom in my ear that says: "Dr. Andro must be mistaken, when he said he saw improvements in my delts and lats... I mean, I did not cut calories and you do...", you know this antics, don't you?

... don't forget to revisit the effect of the tweaks you have made ...

The well-thought changes I made to the exercise side of the equation last week (you may remember that I felt like being at the verge of overtraining), on the other hands, are really paying off. I feel that my regimen is on point! My body responds great to the weekly alternations between EDT, 5x5 and reverse pyramids. As the video on the right goes to show, I did yet incorporate another "body weight" exercise after realizing how well the weighted chins worked for me: Weighted dips!

Video 1 (click to watch): Weighted wheelchair version of the Gironda Dips... I usually do these at the end of my chest & triceps training; instead of doing 5x5’s I just do 5 sets till absolute failure.
Moreover, I did expand my outside of the last weeks of a prep non-existant ab-training to a few minutes after every training session. As I have mentioned previously I am a firm non-believer in the power of isolated ab-work, but in order to make sure I did not miss the slightest opportunity to improve my physique, I better invest the additional few minutes, now, to avoid asking myself later, whether it was the missing ab-work due to which I missed my pro-card.

A much more important factor, in view of achieving optimal conditioning, which is also a recent addition to my routine, is the HIIT "cardio" training I have been doing on my off-days, last week. That has been working fine and I am going to try doing the same protocol fasted, next week... if you wanna know how that went, I'd suggest you mark Thursday, 23, 2012 in your schedule ;-)

... and never forget to get rid of the junk (-food ;-)!
   
If you take a closer look at the usual choices of carbohydrate sources I am consuming on my re-feeds every Sunday (image 3), I suppose that at least some of you will be shocked... a lot of processed (partly also junk-)food. And no, nothing I would ever recommend you should eat on a daily basis, if your goal is to stay healthy, let alone if you are trying to lose weight and/or improve your body composition.

As you can see from the pictures on the right and from other previous blogs where I talk about re-feeds; that I’m obviously a Fat Boy at heart!  But now I’m actually having a change of heart with my carb sources. This past Sunday I was really craving fruits for some reason. Whether it was fresh or dried fruit, I knew that I wanted to gorge on fruit. So after church, I past by my local Trader Joe’s and bought all the fresh and dried fruits I could possibly get my hands on… from bananas, dried bananas and banana chips to mangoes, strawberries and dates, you name it, I bought it!

With the majority of my carb source coming from fresh and dried fruit while keeping the highly processed junk food such as the pancakes and English muffins to a minimum, I felt a lot more refreshed than after the my previous junk-bindges... 2 cans of coconut water later, and I felt in heaven ;-)

In view of the fact that it common wisdom tells you that "feeling in heaven" is not seemly for someone preparing for a bodybuilding contest, I should not forget to add that the one thing that felt even better than the food, was the way my physique responded to the combination of fresh and dried fruit. I have made a habit of practicing my poses about 1 hour after my 1st re-feed meal of the day to see how my physique looks and judge the progress I have made over the last 7 days... and I’d have to say, I was very pleased with what I saw this past Sunday in the mirror... not to mention the awesome pumps and the tight contractions upon flexing the muscles.
Recipe of the week:  
Tropical Papaya Chicken Salad

Ingredients:
  • 12 oz chicken breast - chopped and marinated in fresh pineapple juices, sea salt, pepper & curry powder
  • 1 cup of sliced papayas  - sliced in squares
  • 1 whole cucumber - sliced in circles
  • ¼ cup of real coconut milk  -the one in the can
  • 1 packet of TruviaMacros: 72g protein/ 14g carbs/ 16g fat
How to prepare it: Mix, and toss, then eat and enjoy!
Dr. Andro and I have been talking about reloading glycogen stores before and I remember he mentioned that a 2:1 ratio of glucose to fructose has been shown to do the job just about as well, if not better than your usually overkill of dextrose and/or maltodextrin... I guess, my body must have remembered that and thusly sent me those "fruit cravings" - and what should I say, it certainly felt like it worked. For the future I will thusly be ditching the pancakes and English muffins, to make room for more fresh, ripe and dried fruits and home-made baked potatoes and yams for a more sustained influx of non-fructose carbohydrates to really top those glycogen stores off once in a week.
